Liability Releases

If the participant is under 18, a parent or legal guardian must complete this form and agree to the releases on the participant’s behalf.

General Liability Release:

Participation in any sport, including dodgeball, involves inherent risks, including the possibility of accidental injury, despite all safety precautions. Please carefully consider these risks before voluntarily registering for or participating in any Rose City Dodgeball Association LLC (RCDA) league, tournament, or event.

Photography Release:

I grant permission to Rose City Dodgeball Association LLC (RCDA), its representatives, contractors, and affiliates to photograph, record, and otherwise capture my image, likeness, voice, and participation during RCDA leagues, tournaments, and events.

Code of Conduct:

By registering for any Rose City Dodgeball Association (RCDA) league, tournament, or event, I agree to the following community standards. If the participant is under the age of eighteen (18), this agreement is acknowledged and accepted by the participant’s parent or legal guardian on the participant’s behalf.
